Admission and Enrollment

The following items must be completed and returned to the center by the first day of attendance.
* Child Care Enrollment Form                           * Transportation Permission Agreement

* Age appropriate Intake Form                          *Parent/Provider Agreement and Policies (Handbook)

* Health History and Emergency Care Plan    * Alternate Arrival/Release  Agreement    

*Parent/Provider Payment Agreement             *Permission to Photograph   

*Federal Food Program Forms

The following items must be completed and returned to HSCC within 30 days of enrollment:

* Child Health Report       * Day care Immunization Record

The following items will be provided by parents for their children:
* Diapers                                                                                     

* 2 Full sets of clothes appropriate for the season               * Pack and play (child under 1 year of age)                       

* A pair of indoor only closed-back shoes/slippers            * Sleeping bag with zipper (child age 1 year and up)          

* All necessary weather gear per season

Rates and Payment Schedules:

* All payments are due on Friday for the following week of care. All payments are due in full each week regardless of attendance. If a payment is late a $25.00 fee will be charged and payment must be made before your child can return to care.

* Full-time is 4 or more days per week regardless of the number of hours utilized.

* Your hours must be in writing 1 week prior to (and subject to availability) start date and 1 week prior to any changes in needed child care hours.

* Part-time is 3 days or less per week regardless of the hours utilized.

* No Payment or fee will be prorated. Our rates are based on slots not hours per week. Our rates are the same regardless of the hours your child is here.

* Full Time: 195.00/Week
